RELEASE OF INFORMATION. User hereby requests and authorizes AFFORDABLE FAMILY RIGHTS, and any other holder of information relevant to the legal service to be rendered to the User, to release information upon request, to AFFORDABLE FAMILY RIGHTS, or any other legal services provider. User also authorizes AFFORDABLE FAMILY RIGHTS to review user’s medical and financial history for the purpose of providing legal services hereunder. AFFORDABLE FAMILY RIGHTS represents and warrants that AFFORDABLE FAMILY RIGHTS shall during the term of this Agreement remain compliant with the federal Health Insurance Portability and Accountability Act (“HIPAA”), 42 USC ‘ 201 et seq.
